Patient Intake form

When you schedule you will receive a link to join Healthie.com. Please sign in, select a password, and you will then have access to your intake forms. These forms will help Heather get to know you and why you are coming in.  She cannot see you without your full agreement to her policies or consent to provide treatment.  So be sure to complete all sections of the form.

Get Ready for your Session

Make a list of symptoms or questions that you wish to discuss with Heather when you come in.

Come to your session prepared with loose fitting clothing, jog bra for the ladies, loose fitting pants or shorts if appropriate.  

It may be necessary to have you disrobe in order to address the affected area.  You will be draped if this is required.

Need to Know

How long will you be there?

 Evaluation and treatment sessions are 60 minutes in length.

Appointments will start on time! If you arrive late for your appointment, there may not be a chance to make that time up depending on the schedule. Please plan to arrive early so that you can prepare for your treatment without rushing. 

Payment methods

Heather does not participate in any insurance networks, Medicare, or Medicaid.  This means that you will need to provide payment for your treatment as you go.  

Heather will accept cash or credit card payments.  NO CHECKS.  

You may also use your HSA (Health Savings Account) card and if requested at the time of service, Heather is happy to provide you with a superbill that you can submit to your insurance, but you must let her know at the beginning of your treatment.
